Introduction: Aging is considered a dynamic process, in which morphological and physiological changes occur at all levels of the body. These changes increase the risk of disease, frailty, falls, and sarcopenia, a progressive and widespread musculoskeletal disorder characterized by low muscle strength and mass, which can result in functional decline and disability in the elderly. To mitigate this problem, regular exercise is widely recommended. Both the Pilates Method and weight training can affect sarcopenia indicators. However, the literature still needs to determine the associations between sarcopenia indicators and the practice of these modalities in the elderly, assessing the specific differences between them. Objective: To evaluate sarcopenia indicators in elderly women who practice weight training and the Pilates Method. Methods: This was a cross-sectional study of women aged 60 or older who practiced weight training and Pilates. The volunteers underwent anthropometric assessments and answered the SARC-F questionnaire to identify increased risk for sarcopenia. They also underwent the Handgrip Test and Isokinetic Assessment to assess knee flexor and extensor muscle strength. They also underwent muscle ultrasound of the vastus lateralis and rectus femoris muscles, body composition assessment, and Relative Musculoskeletal Index (RMSI) by Dual-Energy X-ray Absorptiometry (DXA). Finally, they underwent functional tests. Results: The PT group showed significantly higher values for weekly training frequency, vastus lateralis area and thickness, isokinetic flexion torque, and RMSI (p < 0.05). After adjustment by ANCOVA, the vastus lateralis area and handgrip strength remained higher in the PT, indicating independence from time and frequency of practice. Large effects were observed for weekly frequency, vastus lateralis area, and thickness. moderate effects on rectus femoris area, grip strength, flexion torque, RIME, appendicular muscle mass, fat-free mass, and calf circumference; and small effects on rectus femoris thickness and extension torque. Conclusion: Weight training showed a greater impact on muscle mass and strength parameters in elderly women, especially in the vastus lateralis area and handgrip, when compared to Pilates. These results reinforce the importance of resistance training as an effective strategy for the prevention and management of sarcopenia in older women.
