In recent years, Pilates exercises have been one of the most sought after modalities both to increase physical conditioning and for rehabilitation, and its use is common in cases of postural misalignments. However, there is a scarcity in the literature in relation to studies with high methodological rigor that investigate this topic. OBJECTIVE: Check controlled and randomized trials regarding the effects of Pilates exercises on postural alignment METHODS: A systematic literature search and statistical meta-analysis procedure were carried out. RCTs that used Pilates exercises as an intervention and assessment of postural alignment as an outcome were considered eligible. The searches were carried out in the following databases: PubMed; BASIS; CENTRAL; CINAHL; Web of Science; LILACS; SportDiscus and PEDro. The assessment of risk of bias and methodological quality was carried out using the PEDro scale. The quality/certainty of the evidence was observed using the GRADE system. For meta-analysis, the effect measure was a difference in the standardized mean between the groups at the post-intervention time. Heterogeneity was quantified by the I2 statistic and the Cochrane Q test. Values referring to the treatment effect were considered significant when p < 0.05. All analyzes were performed using the RevMan program, version 5.4. RESULTS: Initially, 923 records were located. After the screening process, 15 RCTs were included in the systematic review. No filter was used to limit the year of publication. The RCTs included were published between the years 2012 and 2023. The assessment of methodological quality stated that of the 15 studies included, seven were of high quality (PEDro score ≥ 6 points), with an average score between the studies of 5.6 points . The qualitative analysis demonstrated dispersion between the RCTs, with regard to the approaches and dosages used. There were no significant results for any of the stages evaluated. The assessment of the quality of evidence by the GRADE system was considered low or very low. Furthermore, it was not possible to carry out sub-analyses to check other stages, as there were not enough studies for comparisons. CONCLUSION: As in all RCTs the body segments analyzed demonstrated a low or very low quality of evidence, and no significant results in favor of Pilates were observed, to date, health professionals should use Pilates exercises with caution when the objective is linked to postural improvement, until new RCTs with high methodological rigor on this topic are developed.
